The first results from having 2 gigabytes (GB) of RAM and a nVidia nForce EHCI controller while the latter from a rare issue in the Usbhub.sys driver. The issue occurs when a USB device driver attempts to enter a selective suspend state when a previous attempt was canceled unsuccessfully.

A hotfix has been released by Microsoft which fixes the issues described above. The hotfix can be obtained via one of the following links depending on the operating system:

Windows Server 2003
